Many moral judgments involve three separate questions: What did a person mean to do? What did they actually do? What happened afterward? These can point in different directions. A doctor may take a calculated risk to save a patient but cause unforeseen harm; a company may donate generously mainly to improve its image; a leader may make a careful decision that later produces disastrous results. The challenge is deciding whether morality evaluates character, choices, or outcomes.

Consequentialist thinking gives greatest weight to the effects an action produces, while duty-based approaches often stress principles, intentions, and the limits on what people may do to others. Everyday judgment is usually less tidy. We may forgive an honest mistake yet criticize negligence, especially when someone ignored a predictable danger. We may also admire a beneficial act while doubting the motive behind it. The discussion becomes sharper when responsibility is unequal: leaders and professionals often have more information, power, and a stronger duty to anticipate risks. A useful distinction is between an unintended result, a foreseeable side effect, and harm accepted as a means to an end.
